You can download a prefilled petition by clicking on the link that says "Click here to send prefilled petition emails..."   We recommend printing out the prefilled petition.  

 

You can also download a blank copy of the petition from the website and print it out.   If you do that, then please fill out the petition and turn in to Mary Owens at our office (611 SW Federal Hwy, Suite K1, Open M-F 9am-1pm).  She has a 10-day window after you sign the petition to get it to the SOE office, so please remember to turn in the petition promptly after signing.
